Forms for Reporting Positive Cases, Illness, & Exposure to COVID-19

All COVID-related illnesses and exposures must be reported. Employees and clients must comply with orders of local and state health officers.

These forms are specifically intended for the medical community.

Employees: new positive test, exposed to someone with COVID, or sick with symptoms.

Updates to previously submitted case (ie. change in symptoms & new test results)
